Students will need to create an account and upload a current physical after May 1st, 2022 for the School year of 2022-2023. Also you can find the EGUSD physical form on the mthsmustangs.com under the Forms page. If you have any questions about clearance please email David Coronado, [email protected]. Reminder that Fall Sports start on July 25th for football, August 1st for all sports except for water polo which starts August 15th. Please always make 2 copies of the physical give one to the coach if they ask for it and keep one for your records.

ATHLETIC ELIGIBILITY
Every student who wants to participate in a sport must complete and have signed by a parent/guardian the following forms: 1. Physical / Medical Insurance (Mandatory) 2. Emergency Information (Mandatory) 3. Agreement for Team Participation (Mandatory) 4. Student Alternate Transportation Form (Mandatory) 5. Sudden Cardiac Arrest Form (Mandatory) 6. Concussion and Head Injury Information Sheet (Mandatory) 7. Student Personal Automobile Use Form (Optional) Athletes must have a yearly physical from a qualified physician who completes the medical examination report. In order to be accepted for athletics, the physical must be completed after May 1 of the school year in which the athlete plans to compete. Completed papers should be given to the head coach/athletic director depending on the site’s athletic department policy.
UPDATE
Any student/athlete that was in 9th, 10th, or 11th grade last year and had a physical to participate in sports in 2019/20 does NOT have to get a physical for summer.
* Any incoming freshmen or anyone transferring in from out of the district that we don’t have a physical on file for NEEDS a physical.
* All athletes going to participate in athletics during the 2020/21 school year will need a physical and must be cleared for their season of sport before they can tryout or compete.
